The West Virginia Department of Education (WVDE) Office of Special Education is pleased to provide parents of students receiving special education and related services the opportunity to complete a survey on parent satisfaction with the efforts their school provide. Your input as a parent is very important and will help parents within all of West Virginia.
You should have received your survey at the beginning of May. WVDE has partnered with Measurement Incorporated to help with the survey administration process. You have three different ways of completing the survey:
complete it and return it in the postage paid envelope directly to Measurement Incorporated, or complete it online by scanning the QR code in the top left corner of your survey, or o complete it online by visiting http://mievaluation.com/wvps2023.html. You will be prompted to select the appropriate survey for your child (preschool or school age), and to enter the survey ID that appears below the survey QR code.
If you have misplaced or lost your survey ID, you may fill out and submit the “Parent Survey Request Form” by visiting https://measurement.welcomesyourfeedback.com/s/WVPS-Request. A representative will contact you to provide your Survey ID.

Students entering 7th or 12th grade must have all required vaccines before they will be able to enter school this fall. If you were sent a letter that your child needed vaccines, and you want the vaccines administered by Grant County Health Department nurses at school, please send permission forms tomorrow March 22, 2023.

We are excited to have the opportunity to once again support our PHS Vikings Girls Basketball Team as they play in Charleston for a state title. If your child is attending the game and they are not riding the pep bus, in order for their absence to be excused they must bring their ticket stub from the game to school when they return. The ticket stub should be submitted in the same manner in which they turn in an absence note. If your child is riding the pep bus they do not need to submit a ticket stub.
